We are currently seeking a highly motivated Advanced Nurse Practitioner to join our practice team. The role will encompass community home visits along with consultations with patients in practice whether they be acute on the day appointments or multi co morbidity health reviews. The role we are offering will require the Advanced Nurse Practitioner to be autonomous in all aspects of role whilst having the clinical support of a full multi healthcare professional team. Daily communication meetings along with monthly clinical area meetings support the role and we look for clinically confident individuals to join the team. You will see a range of patients including children, presenting with undifferentiated diagnoses to assess, treat and prescribe, referring as necessary where appropriate using practice clinical pathways. 

Based within the semi-rural village of Longton in Lancashire together we work as a friendly, high performing and forward thinking practice, with a current list size of over 10500 patients. We work together as a training practice and also accommodate medical students from both UCLAN and Manchester Deanery. We are proud to be a part of Preston & South Ribble Primary Care Network which also includes another three local practices that meet regularly and have developed a network Nursing forum which provides both networking and training opportunities and also peer clinical and wellbeing support. 

The post is for between 30 - 37.5 hours per week over and will include a Wednesday, Thursday and Friday as days of work however we will always consider both part time and full time applications and are happy to look at options and hours that would suit the individual and practice needs, considering any carer responsibilities or needs. Salary commensurate with qualifications and experience. 

The successful candidate will be a fully qualified Advanced Nurse Practitioner and Non- medical prescriber (V300) with previous experience in working in Primary Care preferable but not essential. Additional interest in the management of the frail and elderly is desirable but not essential.
