To address the ever-increasing demand for health care services and to better support individuals within their own communities, there is wide recognition of the need to embrace new ways of working and digital healthcare technologies to ensure all patients receive the right care, in the right place, at the right time. Additionally to this, data can play a key role in the shaping and delivery of PCN and wider healthcare services. The practice based role of the Care Coordinator – Admin, Data & Digital will be to support patients and healthcare professionals to maximise the potential presented by new digital healthcare technology and the associated new ways of working, these include: • Healthcare Apps • Wearables and remote monitoring technology • Data and intelligence – e.g. Population Health data, Aristotle Digital & data care coordination is a mechanism for delivering 21st Century care and connecting people with both medical and non-medical sources of support, empowering people to better self-manage their conditions and/or work collaboratively with healthcare professionals to manage their conditions. In terms of data, the role will be to support the delivery of services, shape and grow current and future services, help secure funding based on population need and health management, support PCNs and practices with projects (such as shared programmes as stipulated by local health partners (ICBs) and national directives (NHS England) etc. The post holder will be confident and comfortable with the use of digital healthcare technology and local data and intelligence systems Have the ability to work as part of a team and to support people in a way that inspires trust and confidence, motivating others to reach their potential. Working towards or have attained NVQ Level 2 Business Administration (or relevant experience).
