William Hopwood Surgery are currently looking for a full-time Quality & Improvement Lead to join our dynamic and evolving organisation. Previous experience of working in a busy general practice environment is preferred.

Main duties of the job: The successful candidate will be required to work as part of a Team with opportunity to bring in their own initiatives and ideas. 

• To work with the Practice Leadership team to identify, implement and review the systems within the Practice, to improve patient outcomes and maximise outcome points. 

•To utilise Practice and patient data to improve performance and assist the Practice in meeting targets and improving the provision of health to the Practice population. 

Job description 

Promote national, local and practice quality standards for chronic disease management, to ensure that the quality of care is maintained at a high standard using NICE guidance and the existing GP contract as a guide. Work with senior leadership team to develop strategies for improving patient outcomes and maximising points for the Practice using required quality indicators like QoF/IIF and enhanced services – both local and national. Utilise available data and quality improvement strategy to identify patients requiring long term condition and QoF reviews and ensure this process runs efficiently. 

These include: 

• Assist with maintaining accurate disease registers within the clinical system (EMIS) 

• Running monthly reports for individual QOF domains and contact the patients using SMS, telephone, email or letter to ensure contact is made within the correct time scales and the correct read codes are used for reporting purposes. 

• Running a monthly report for all enhanced services, including high risk drug monitoring recalling patients as above. 

• Running a monthly report and inviting patients in for their NHS health check. 

• Running monthly CQC searches, completing any action required and updating the monitoring spreadsheet, highlighting any issues to the senior management team. 

• Supporting the Practice manager in the submission of month end/year end data. 

• Organising and recalling of eligible patients for vaccination clinics/home visits. 

•Proactively seek and implement a variety of quality and improvement projects, including those directed by the senior leadership team. 

•Where required to assist and undertake clinical and administrative audits 

•To check and correct patient coding against business rules for GP contracts i.e: QOF/ IIF/ QOEST 

•Work with clinical and non-clinical staff to improve the accuracy of data input 

• Assist with maintaining accurate disease and QoF registers within the clinical system 

• To proactively contribute and par take in any quality improvement activities and opportunities that Practice management team proposes ( including review of significant events and complaints in line with Practice policy) 

Person Specification 

Experience Essential 

• An understanding of EMIS including running clinical searches 

• Sound understanding of quality and improvement tools like QOF /IIF /QI 

• Good communication and organisation skills 

• Experience of working in a team ideally GP Practice 

• Good analytical skills 

• IT skills 

• Ability to work under pressure and to deadlines 

• Ability to prioritise and track own workload Desirable 

• Ability to positively influence Practice team members to work towards maximising achievements and quality improvement goals 

• Willingness to learn and be part of a friendly Practice team 

• Being flexible to support other team members as and when needed 

• Project management skills 

• Microsoft excel skills 

• A background of working in Primary Care Disclosure and Barring Service Check
