Fishergate Hill Surgery - Practice Nurse

Due to retirement, Fishergate Hill Surgery is looking to recruit an experienced and motivated Chronic Disease Practice Nurse to join our friendly nursing team. The nursing team comprises of an Advanced Nurse Practitioner, Registered Nurse, Health Care Assistant, a Clinical Pharmacist and a PCN Pharmacist. The post will be part-time working 24 hours per week Tuesday, Wednesday and Friday. The post holder will provide and maintain a high standard of chronic disease management for patients with chronic diseases (especially respiratory and diabetes), which includes the diagnosis, monitoring and development of patient’s individual management plans and agreeing these as appropriate with the patient and other health professionals.
