We are very excited to be recruiting two nurse associate roles for the Burnley East area. A knowledge of PCNs and what we are trying to achieve is a must in this role. We are looking for someone who is passionate about the role of the nursing associate and the opportunity work with a multi-disciplinary team to deliver patient care. We are looking for someone who is flexible, self-motivated, willing to embrace change and be proactive in continuously improving service delivery. 

The Nurse Associate will: 

• Be responsible for delivering high quality, patient centred, compassionate care under the direction of a Community Link Nurse with a focus on promoting health and independence 

• Be an accountable professional 

• Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual 

• Carry out specific clinical and care tasks and responsibilities to a high standard and competency, under the direction of a Community Link Nurse 

• Have the ability to work without direct supervision, at times delivering care independently in line with the individual’s defined plan of care, within the parameters of practice of the Nurse Associate role, accessing clinical and care advice when needed. 

The responsibility of the postholder is to participate in the planning and delivery of the Enhanced Health in Care Homes model of proactive care that is centred on the needs of individual residents, their families and care home staff. They will work independently under the supervision of the Community Link Nurse, providing holistic evidenced based high quality care to patients and will be professionally accountable for their actions.
