Main duties of the job 

Work within a nationally recognised medicines management team (The Pharmaceutical Journal, September 2017). Help to build and deliver Pharmacist-led clinics, utilising your skills and experience to provide our patients with specialist knowledge and expertise to manage their conditions. Provide medication support, advice and guidance to Patients, GP colleagues and the wider MDT. Complete medicines reconciliation following hospital discharge and appointments in secondary care. Take part in education and training (including delivery of learning sessions where appropriate). As part of this vacancy we will consider applicants who wish to work either full or part time.
