Practice Nurse Accountable to: Partners of the Practice Reports to: Dr M Siddiqui Liaises with: Patients and Users of the Practice Medical and Nursing Teams Reception and Administration Staff Outside Agencies Purpose and Objectives of the Role The post holder is responsible for the management of nursing service delivery to the whole practice population; both the management of long-term conditions and the provision of preventative health care. The nurse will: work autonomously, managing own workload within the limits of the role, demonstrating clinical expertise and skilled decision-making in practice; work collaboratively within the general practice team to meet service needs; lead in the development and implementation of practice protocols; lead in clinical audit and utilise benchmarking to ensure best practice. Key Responsibilities Clinical • Lead in the management of treatment plans of Chronic Disease reviews appropriate to each patient; demonstrating proficient needs-based assessment, evaluation skills and medication reviews. • To eventually manage and maintain chronic disease registers and work with admin staff to construct and maintain robust call and recall systems for chronic disease, cervical cytology, immunisation and other population based health promotion and disease care programmes. • Prioritise health needs and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. • Manage service delivery to meet the needs of patients presenting for opportunistic care. • Manage nursing care delivery ensuring evidence-based practice in accordance with statutory guidance. • To provide basic and higher levels of nursing care for patients. • To provide care for minor injuries, dressings, removal of stitches and general wound care. • To carry out cervical cytology sampling, immunisations (including running baby clinic) as outlined by the national programmes and local guidelines, working closely with the admin team to create systems to identify patients who are in the need of such searches and promoting the service for the hard to reach patients. • To support and eventually lead the annual flu campaign and other mass population vaccination programmes as needed in line with national guidance. • To conduct family planning consultations. • To carry out health checks in conjunction with national, local and practice protocols, setting up and maintaining a system for identifying patients in the target group for such checks. • To eventually establish and participate in a robust system of clinical audit in partnership with other clinical and administrative colleagues, leading in areas appropriate, such as infection control. • To counsel patients on the meaning of test results and other medical information. • To provide support and guidance to patients in times of crisis. • To provide support to the GP’s in management of chronic diseases such as diabetes, COPD, asthma, hypertension, etc. • To provide the provision of a travel advice and vaccination service in accordance with practice protocols. • To act as chaperone or similar support for other colleagues as needed. • Encourage the support of patients to adopt health promotion strategies and apply principles of effective self-care as well as primary prevention of cervical cytology. • To eventually lead, in partnership with other clinical and other management staff, the necessary data collection for QOF and other incentive schemes that is needed by the practice, prompting other members of the team as needed in the Practice. Communication • Communicate effectively within the practice team and beyond upholding the good reputation of the practice and maintaining professional standards • Communicate effectively with patients recognising potential barriers to communication and the need for alternative methods to address different levels of understanding, cultural background and preferred ways of communicating Delivering a Quality Service • Recognise and work within own competence and scope of practice; maintain membership of governing regulatory body (NMC) and professional organisation (RCN); adhering to the code of conduct and demonstrating professional standards. • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures. • Prioritise, organise and manage own workload effectively. • Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation. • Participate in the maintenance of quality governance systems and processes across the practice. • Collaborate on improving the quality of health care in partnership with other clinical teams, responding to local and national policies and initiatives as appropriate. • Evaluate the patients’ response to health care provision and the effectiveness of care. • Support and participate in shared learning across the practice and wider organisations. • Identify learning from clinical incidents and near-miss events using a structured framework (e.g. root-cause analysis). • Assess the impact of policy implementation on care delivery. • Participate in the performance monitoring review of the team, providing feedback as appropriate. • To have an awareness of what constitutes a Safeguarding concern and if one arises to know the appropriate individual or organisation to contact. Leadership – Personal and People Development • Take responsibility for own development learning and performance including participating in clinical supervision and acting as a positive role model. • Support the development of others in order to maximise potential. • Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice. • Encourage others to make realistic self-assessment of their application of knowledge and skills, challenging any complacency and actions that are not in the interest of the public and/or users of services. • Work with practice management to ensure sufficient staff of appropriate ability, quality and skill-mix are available to meet current and future service delivery General/Administrative • To maintain your own annual registration with the NMC and adhere to its Code of Professional Conduct. • Maintain any other required registrations and certifications necessary to fulfil your role. • To promote a professional approach at all times by dress and attitude to patients and staff both in and out of the practice, setting an example for those under your direct authority and to the team as a whole. • To maintain your own continuous CPD and ensure a PDP is kept up to date in line with the practice development plans. • To participate in and attend practice meetings and any in-house training. • To maintain accurate patient records, entering onto the practice system (Emis Web) the appropriate codes, using agreed standards of data entry, templates and protocols, completing all documentation, computerised and paper-based, that is required for good patient care. • To eventually lead, in partnership with our clinical and managerial team, the necessary data collection for QOF and other incentive schemes as needed by the practice, prompting other members of the team as needed for assistance in gathering the required data. • To create and maintain in cooperation with other nursing and admin colleagues a system of clinical stock and equipment maintenance, calibration, and supply; working closely with the practice manager to ensure cost efficient purchasing of perishable and expensive items. You may be required to perform other duties from time to time to meet the needs of patients and the practice Learning and Development • Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments. • Assess own learning needs and undertake learning as appropriate. • Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning. Management of Risk • Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients. • Monitor work areas and practices to ensure they are safe and free from hazards and conform to Health and Safety and legislation, policies, procedures and guidelines. • Ensure safe storage, rotation and disposal of vaccines and drugs. • Support members of the nursing team to undertake mandatory and statutory training requirements. • Apply infection-control measures within the practice according to local and national guidelines.
