This is an exciting role in General Practice combining patient-facing, analytical and administrative elements of work. 

The successful applicant will be working in a large coastal practice which is forward thinking and dynamic. Holland House Surgery serves a population of around 11,000 patients in the beautiful and thriving town of Lytham. We also have a satellite practice in Freckleton. 

You will be well supported by our friendly and helpful multidisciplinary team consisting of GPs, Advanced Practitioners, Nurses, Healthcare assistants, Physicians Associate, senior clinical pharmacist, PCN pharmacist and pharmacy technician. 

We are looking for an extra clinical pharmacist to join our existing pharmacy team to help deliver our medicines optimisation goals. The role will be challenging, varied and will include: 

  • Conducting face-to-face and telephone medication reviews with patients.
  • Answering medication related queries from patients and other clinicians.
  • Optimising medicines to manage chronic diseases.
  • Managing acute medication requests.
  • Medicines reconciliation.
  • Audit work.
  • Liaising with our established PCN pharmacy team.
  • Participating in virtual and in-person group consultations.

You will be supported in learning and professional development through regular practice team meetings on a variety of topics. Although an existing prescribing qualification is desirable, support to achieve this can be provided. Some experience of general practice systems would be beneficial but applications from all sectors will be considered with plenty of support to develop into the role. 

There is flexibility to develop the role according to your skills, experience and interests in line with the needs of our practice and patients. 

We are looking for someone to work for 3-5 days a week at the surgery. There is currently no weekend working.

Essential 

Qualified Pharmacist registered with the General Pharmaceutical Council 

Desirable 

Independent Prescriber or working towards this.

Experience of working in general practice or general practice IT systems

Clinical Diploma
