Clinical Pharmacist
Salary dependent upon previous experience
Working week – All day Wednesday to Friday Possibility of working one of the days from home.
Job summary
We are looking for an enthusiastic, committed and motivated Clinical Pharmacist to join the team and are seeking someone who ideally has previous experience of working in general practice, or alternatively experience of working in hospital, primary care, community or mental health pharmacy settings for at least 2 years post-qualification, to work with our existing clinical pharmacist to develop and lead medicines optimisation services within our practice. The successful candidate will have a patient facing role in managing long-term conditions; working directly with patients to assess and treat conditions, as well as promote self-care. You should have excellent written and verbal communication skills, experience of influencing others and conducting clinical medication reviews. You will work as part of a multidisciplinary team with clinical support to perform face to face and telephone reviews to optimise medicines for patients with polypharmacy. You will manage the practice repeat prescription process, provide medicines information advice, perform medicines reconciliation upon transfer of care between settings and promote systems for safer prescribing. The successful candidate will also contribute to quality improvement and clinical audit. The applicant will be registered with the GPhC, ideally have a post-graduate clinical diploma and be an independent prescriber. If not already an independent prescriber, they should be willing to undertake training to achieve this. Candidates must have strong written and verbal communication skills and an ability to work effectively under pressure. Main duties of the job
The key aims for the clinical pharmacist are:
1. To assist in the medicines related workload with prescribing colleagues.
2. Increase safety and quality of prescribed medicines
3. Provide support on medicines related issues to the whole practice team
4. Be a point of contact for patients regarding medicines queries
They should support the following clinical work:
1. Clinical Medication Review
2. Chronic Disease Management (for example, managing hypertension/Type 2 Diabetes/Asthma/COPD patients, polypharmacy review)
3. Use appropriate provided clinical templates within the EMIS Web Clinical System
4. Medicines reconciliation on transfer between care providers
They should also support the following practice development work: Prescribing Safety, Clinical audit, Local Enhanced Services support Lead on prescription management (systems and processes, ensuring monitoring is being done, supporting clerical staff) We offer excellent terms and conditions including annual leave of between 27-33 days per year plus Bank Holidays pending continuous NHS service, free onsite parking, uniform, access to ongoing training and development.
Person Specification
Qualifications Essential
• Mandatory registration with the General Pharmaceutical Council
• Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
Desirable
• Membership Primary Care Pharmacy Association (PCPA)
• Membership of the Royal Pharmaceutical Society
• Minimum of two year’s experience as a pharmacist, demonstrated within a practice portfolio Experience Essential

• Experience and an awareness of the breadth of common acute and long-terms conditions that are likely to be seen in general practice
• Demonstrates ability to integrate general practice with community and hospital pharmacy teams community groups
Skills and knowledge Essential
• In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
• Knowledge of IT systems, including ability to use word processing skills, emails and the internet to create simple plans and reports
• Able to obtain and analyse complex technical information.
• Understand the principles of research governance
• Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face
• Excellent interpersonal, influencing and negotiation skills, organisational skills with the ability to constructively challenge the view and practices of managers and clinicians
• Effective time management (planning and organising)
• Demonstrate personal accountability, emotional resilience and work well under pressure
• Able to provide leadership and to finish work tasks
Desirable
• Good clinical system IT knowledge of EMIS
• Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities
• Able to plan, manage, monitor and review general medicine optimisation issues in core areas for long term conditions
