Main duties of the job
To provide local leadership to the Practice admin and workflow teams whilst contributing positively to the overall improvement culture within the Practice. To work with the Practice Leadership team to identify, implement and review the systems within the Practice, to maximise income and patient outcomes. To utilise Practice and patient data to improve performance and assist the Practice in meeting targets and improving the provision of health to the Practice population.
Job description
Work with senior leadership team to develop strategies for improving patient outcomes and maximising income for the Practice using QoF and enhanced services – both local and national. Promote national, local and practice quality standards for chronic disease management, to ensure that the quality of care is maintained at a high standard using NICE guidance and the existing GP contract as a guide. Utilise available data and quality improvement strategy to identify patients requiring long term condition and QoF reviews and ensure this process runs efficiently.
These include: Supporting the Practice manager in the submission of all quality domains like QOF, LES, month end/year end data. Running reports and inviting patients in for their QOF, LES NHS health check appointments. Running monthly CQC searches, completing any action required and updating the monitoring spreadsheet, highlighting any issues to the senior management team. Supporting the Practice manager in the submission of month end/year end data. Supporting the Practice manager in PPA claims. Organising and recalling of eligible patients for vaccination clinics/home visits. Proactively seek and implement a variety of quality and improvement projects, including those directed by the senior leadership team. Where required to assist and undertake clinical and administrative audits To check and correct patient coding against business rules for GP contracts i.e: QOF/ IIF/ QOEST Work with clinical and non-clinical staff to improve the accuracy of data input To take a lead and develop the Practices use of patient online services. Organise and order vaccination and immunisations stocks. Assist with maintaining accurate disease and QoF registers within the clinical system To manage the complaints and significant events processes in line with Practice policy, proactively linking any actions with quality improvement opportunities To assist with premises maintenance and identify improvement opportunities as directed by the Practice Manager To assist with health and safety risk assessments, identifying and acting on any actions as guided by the Practice Manager
Person Specification
Experience Essential
• A background of working in Primary care
• Strong understanding of quality and improvement tools like Qof / IIF
• Strong communication skills
• High levels of organisation
• Experience of working in a team ideally GP Practice
• An understanding of EMIS, including running clinical searches
• Evidence of being able to handle data
• Strong IT capability
• Ability to work under pressure and to deadlines
• Ability to prioritise and track own workload Desirable
• Project management skills
• Microsoft excel skills
