Multi-Disciplinary Team; GP’s, prescribing Pharmacist, Nurse Practitioners, Practice Nurses, HCA’s and Admin, with excellent local knowledge. Excellent admin and pharmacist support to protect your clinical time Daily team clinical discussion for coffee, catch up and peer support. ICB led protected learning time. A member of the Eden PCN with Frailty Co-ordinator, First Contact Mental Health Practitioner, First Contact Physio, Admiral Nurse and Social Prescribers for young people. Purpose built 2005 premises, shared with community services enhancing continuity of patient care, with spectacular riverside location.
