Advanced Clinical Practitioner - Bay Medical Group

Are you an experienced Advanced Clinical Practitioner ready to lead innovative patient care in one of the UK's most progressive primary care settings? Bay Medical Group is seeking an ACP to join our award-winning urgent care team, serving over 54,000 patients in the vibrant Morecambe Bay area. This is a permanent position, working 37.5 hour per week (part time considered). Working pattern must cover a Wednesday and a Friday and the post holder will be expected to work across our 5 sites. We are particularly interested in candidates with advanced skills in frailty assessment/ home visits, cardiovascular risk management, and complex comorbidity care.
