We are looking for a caring, enthusiastic, and motivated Practice Nurse to join our friendly multidisciplinary team. This is an exciting opportunity for an experienced nurse, to play a key role in delivering high-quality clinical services to our patients.
The successful candidate will work closely with GPs, healthcare assistances, and the wider practice team to provide evidence-based nursing care and support the management of both acute and long term health conditions.
Key responsibilities:
- Deliver a range of nursing services including - cervical screening, childhood and adult immunisations, travel vaccinations, wound care and dressings, NHS health checks.
- Support the management of patients with long term conditions including:- Diabetes, Asthma, COPD, Hypertension and Cardiovascular disease.
- Provide health promotion and lifestyle advice
- Assist with infection prevention and control procedures
- Maintain accurate clinical records using EMIS Web
- Participate in clinical audits, quality improvement activities and professional development.
To be successful in this you role you will have:
Essential
- Registered Nurse with current NMC registration
- Experience of working within GP practice
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multi-disciplinary team
- Strong organisation and time management skills
- Commitment to delivering high standards of patient care
- Qualification in cervical cytology
- Experience managing long-term conditions
- Immunisation and vaccination training
Desirable
- Knowledge of EMIS Web
